How the local system is structured
The picture in Health and Social Care remains very complex and the below is an attempt to put across the main changes . We aim to update you regularly on changes as they will have an impact on the Voluntary & Community Sector in Hackney
CCG’s (Clinical Commissioning Groups) may be merging across 7 Boroughs
North East London Health & Care Partnership Formerly North East London STP) covers 7 boroughs and will commission North East London wide health care interventions and campaigns and work on workforce development issues across East London. There are discussions exploring the potential to merge all the CCG’s into this partnership over the next year or two.
Under this partnership sits health and social care delivery systems known as Accountable Care System or Integrated Commissioning Systems –

Is made of a partnership of City & Hackney CCG (for now) London Borough of Hackney, East London Foundation Trust (ELFT), City of London Corporation; Homerton University Hospital Trust and the Voluntary & Community Sector are named as a partner.

Neighbourhoods – Primary Care networks
In Hackney the Neighbourhoods programme has been underway for 18 months. This year the Government required all GP practices to take part in and establish Primary Care Networks. They are getting budgets directly from Central Government. In Hackney the Primary Care networks agreed to align geographically with the neighbourhoods.
There are a number of pilots going on across the neighbourhoods and the VCSE is leading one in Well Street Common neighbourhood that aims to better connect residents to services and activities thye need, connect local VCSE and statutory organisations to each other, and to test and learn re how the VCSE can best organise itself to work in neighbourhoods.

What can Voluntary & Community Sector Organisations do to prepare?
Services will be provided in neighbourhoods, some contracts and provision will still be borough wide, but configured into neighbourhood teams.
Some contracts and provision will be moved to East London Health and Care Partnership level but may require a borough level response.
We believe some funding will be issued on a neighbourhood basis.
If you are a small geographical focussed organisation then the neighbourhoods programme should improve your links with local services and your ability to support local residents.
If you are a small culturally / ethnically /age or condition specific boroughwide organisation, then please get involved with the relevant networks, the VCSE led digital referral project, and soon the revitalised City and Hackney Together consortia vehicle, so we can ensure social prescribers / navigators are aware of your work.
If you are a larger borough wide organisation Join the relevant networks start thinking now how you can reconfigure your services to meet the neighbourhoods configuration. And if relevant to you, start making links with organisations in the North East London Health and Care Partnership. The inner north east london area is Tower Hamlets, Waltham Forest and Newham. Hackney CVS has good links with Tower Hamlets CVS and is working to create links with Waltham Forest organisations.
